How to Obtain the Megalomaniac Diamond in Path of Exile 2?
The Megalomaniac Diamond is a rare drop from Kosis, the Revelation, the hardest Delirium boss at difficulty 4. To fight Kosis, you need to use The Simulacrum at the Realmgate in the Atlas. First, you’ll need to obtain a Simulacrum. You can get one by combining 300 Simulacrum Splinters.
These can be obtained by farming Delirium-instilled Waystones and Delirium nodes. Alternatively, you can buy one through the currency exchange or trade site. To increase The Simulacrum’s difficulty to +4, you need Delirium Atlas points.

You earn these points by completing The Simulacrum at a regular difficulty. Each time you beat the Simulacrum, you get 2 Delirium Atlas points. After you collect enough points, you can raise the difficulty by +1 each time.
After four successful completions, you can increase the difficulty to +4. Only at this difficulty will Kosis, the Revelation drop Megalomaniac Diamonds. These Diamonds are the key to improving your endgame build.
Exploring the Role of Megalomaniac Diamond in PoE 2
A Megalomaniac Diamond has 2-3 modifiers that do one thing: allocate Notables in the passive skill tree. These modifiers are powerful because they automatically assign the Notables passive skill when socketed in a Jewel socket. This removes the need to path toward the skill in the tree.

It works like Anointing an Amulet, but with two or three passive skills at once instead of just one. Since there are many Notables on the passive tree, the value of a Megalomaniac Diamond can be unpredictable. A Megalomaniac with three useful Notables for a meta build could be worth hundreds of Divine Orbs.
On the other hand, if it offers Notables that don’t match a build, it may only be worth a Regal Orb. The Megalomaniac Diamond cannot be modified. It is always corrupted when it spawns, just like the From Nothing Diamond.
This means you cannot use a Vaal Orb or any other method to influence it. Because of this, the only way to get a useful Megalomaniac Diamond is to trade with other players. This makes it rare and highly sought after if it matches the right build.
