These are farmable from Delirium imbued Waystones as well as Delirium nodes. Or you can buy one from a currency exchange or trade site. To raise the difficulty on The Simulacrum to +4 you must have Delirium Atlas points.
You earn these by doing The Simulacrum on a normal difficulty. Whenever you win the Simulacrum, you receive 2 Delirium Atlas points. When you have collected enough points, you can increase the difficulty by +1 each time.
After four tries successfully finished, you can set it to +4. It is only at this difficulty that Kosis, the Revelation drops Megalomaniac Diamonds. These Diamonds are how you upgrade your endgame build.” The Diamonds.
A Megalomaniac Diamond has two to three modifiers that do one thing: they grant Notable passives on the tree. These modifiers are very powerful as they grant a Notable passive skill (which are normally very expensive to path to) when placed inside a Jewel socket. No more walking there in the skill tree.

Since there are numerous Notables on the passive tree, the worth of a Megalomaniac Diamond may be quite unpredictable. A Megalomaniac with three useful Notables for a meta build could be worth hundreds of Divine Orbs.
However, if it has Notables that aren’t related to a build, it might be worth just a Regal Orb. The Megalomaniac Diamond is unalterable. It is always corrupted when it spawns, like the From Nothing Diamond.
This means the item cannot be Vaal Orbed or otherwise influenced. Because of this, the best way to obtain a useful Megalomaniac Diamond is to trade with other players. That makes it quite rare and very much in demand if it fits the right build.
